The Core Distinction

Shock trauma is localized in time and memory, there's a before, during, and after. The nervous system gets stuck in an incomplete survival response around that specific event. Complex trauma is diffuse, there's no clear beginning, its impact lies in identity and relational patterns, not in a specific memory.

In Practice

For shock trauma, methods like EMDR and Somatic Experiencing are often very effective, frequently within ten to twenty sessions. For complex trauma, this is rarely sufficient, NARM, IFS, Sensorimotor, and longer-term relational therapy are more appropriate. The relationship between therapist and client itself becomes a crucial area of work.

Common Misconception

The mistake is treating a complex trauma client as if they have a collection of shock traumas, using the same techniques. The result is often that the client feels some relief in one area, but underlying relational patterns remain unchanged, and new symptoms emerge elsewhere.

Veelgestelde vragen

Can shock and complex trauma co-occur?

Almost always. Someone with complex trauma is also more vulnerable to shock trauma, as their regulatory system is already overwhelmed. Treatment then requires a phased approach.

How do I know which one I'm facing?

Ask about their relational history. With complex trauma, there's chronic insecurity or disruption in early life. With pure shock, the relational foundation is often intact and there's a recognizable pivotal event.

Which training helps me make this distinction?

Trauma-phased treatment according to Judith Herman (stabilization, processing, integration) is a good basic framework. Nearly every trauma training builds upon this.
